Guidelines
Install the App from the AppStore
Enter your Twilio Account SID and TOKEN
In your flow builder, select the Action step and select the "Phone Number Lookup" action under "Integration" block
Select an action "Lookup Carrier" or "Lookup Caller"
Enter the Phone Number
Assign the custom fields in Response Mapping
Run the flow
